SPORT PHYSICALS: if a student-athlete does not have a physical on file from 2019-20, they must complete this form.  The physical must have taken place on or after April 15, 2019 and completed and signed by an MD, DO, PA or NP.  All forms must be completed and approved by the athletic office prior to practices/games.
